Dream Adult Day Care is a community-based long-term care program that provides comprehensive health care with therapeutic, social, educational and recreational activities. The program enhances the lives of the frail elderly, chronically ill and disabled adults while maintaining their residence in the community. We provide a period of respite for family caregivers.

SERVICES.

Participants are offered a wide range of services under the direct supervision of a registered nurse and the participant’s physician. These services are provided or arranged during program hours:

• Nursing care
• Nutritious meals & snacks
• Physical & occupational therapy
• Social worker
• Health education
• Therapeutic activities
• Podiatry
• Barber/beautician services

ACTIVITIES.

To promote social interaction among participants, our Activities Director schedules activities to include:

• Bingo with prizes
• Scheduled movies
• Pingpong tournaments
• Billiard tournament
• Video games
• Mahjong competition
• Nap time
• Board games
• Poker games

ELIGIBILITY.

Dream Adult Day Care is a daytime alternative to
nursing home or homecare services. To qualify,
participants must be:

• Age 18 or over
• Diagnosed with physical or mental impairment
• Not aggressive or abusive
• Free from communicable disease
• Medically stable and able to participate in program
• In need of daily supervision
• Require assistance with personal care

We welcome Medicaid beneficiaries as well as Private Pay.

TRANSPORATION.

We provide door-to-door transportation within 10 mile
radius of the facility. The bus accommodates wheelchairs
and assistive devices.
